LNA - Full Time.  Rotating weekends required. 1-2 years experience preferred. Responsible to the primary nurse for carrying out the duties assigned in order to increase the level of comfort and to maintain personal hygiene and a safe, healthy environment for the patient. Maternal Child Health RN - Full Time Shift: Day shift Hours: 8:00 - 4:30, some evenings or weekends. - 1-2 years experience preferred. As a Maternal Child Health Nurse, you will bring caring, comfort, education and resources to young families as an advocate for the family for services in the Agency and the community at large and by implementing the physician-ordered plan of care, health education, and anticipatory guidance for families. Training/Experience: Must be licensed as a Registered Nurse in New Hampshire and Vermont. Graduate from NLN approved Nursing Program. Two years in Maternal and Child Health nursing in home care or hospital setting preferred, or one to two years of medical surgical experience.

Hospice RN - Full Time Shift: Day shift Hours: $3,000 Retention Bonus Masters Degree Required 3-5 years experience required. This position assists in the resolution of patient and family problems by completing psycho-social assessments establishing goals, and following through with direct treatment and/or referral to community services.  This includes such issues as legal, financial, spiritual, emotional, interpersonal as well as Advanced Directives.  Develops and implements policies and procedures to carry out agency's goals.  Obtains and communicates input from staff regarding efficiency of current policies and need for revisions and/or new policies and procedures to the Patient Care Manager. Finance Patient Account Representative - Full Time Shift: Day shift Hours: 8:00 am - 4:30 pm Contact Information:  Email:   EmailHR@vnavnh.org - HS Diploma or Equivalent Negotiates with third party payers and patients to obtain payments for accounts which are overdue. Monitors Accounts Receivable over 90 days and contacts patients and insurance companies to resolve accounts. Performs follow-up duties for individual branch offices to reduce levels of overdue accounts, which will reduce the receivables and increase cash flow.

- 1-2 years experience preferred. Provide basic personal care, transferring, and assistance with ambulating, housekeeping, meal planning/preparation, and shopping to help clients remain in their homes safely. Must be a high school graduate or equivalent with the ability to work independently and as a member of a team. Must be able to lift, push, pull or carry a minimum of 50 pounds and be able to move the weight a distance of five feet.
 Retention bonus of $250 at 3 months and $250 at the end of the 6th month for those working 32 hours a week (each week) with no unscheduled call outs Earned time accrual for those working 32 hours a week (each week) with no unscheduled call outs for blocks of 10 weeks.  At the end of the 10th week – they will earn one earned time day to request for scheduled time off.

Rehab Physical Therapist - Full-time.  $7,500 retention bonus. Contact Holly Amoth.  Clinical assessment and treatment of patients referred by a licensed physician. Must have license in Vermont and New Hampshire as appropriate.  Includes health and dental insurance, flexible spending, and 403b.

Support Services Scheduler -  Full-time.  Required to work every third weekend. HS Diploma or equivalent.  Provides scheduling support for clinical and paraprofessional teams. Minimum of one year experience in a health care environment.

Triage Supervisor - Part-time.  Every third weekend. 1-2 years experience required. Responsible for triaging calls, managing staff schedules and communicating with referral sources.

Hospice and Palliative Care Registered Nurse (download application)
