Central Vermont Home Health and Hospice - Berlin, Vermont

Licensed Nurse Aide (apply online) - We currently have part-time positions available for LNA's in our Home Care program.

Home Care RN's (apply online) Full-time Vermont licensed Registered Nurses to work in our Home Care Program to provide skilled nursing care in a home setting. Two years med-surg. experience is preferred.

Hospice & Palliative Care RN (apply online) Part-time (30 hour week) Vermont Registered Nurse to work in our Hospice & Palliative Care program to provide hospice & palliative care to patients in their homes.

Maternal Child Health, RN (apply online) Full-time (37.5 hours week) Vermont Registered Nurse to work in our Maternal Child Health Program to work with children and families in their homes. The Community Health / Maternal Child Health Nurse provides direct nursing care to women, infants, and children. The care provided reflects a specialized knowledge, expertise, and appropriate intervention in accordance with prescribed standards of nursing.

Telehealth, RN (apply online) (20+ hour per week)  Vermont Registered Nurse needed for tele-monitor duties; installation, analyze data, programming, etc.

Physical Therapist (apply online)  Full-time. Vermont licensed Physical Therapist to work in our Home Care program, treating individuals of all ages in their homes.

Family Support Worker, Maternal Child Health  (apply online) Full-time/Part-time Family Support Worker needed to work in our Maternal Child Health Program. To provide home and county based support, role modeling and advocacy for families who may be faced with life stressors, such as acquisition of parenting skills and continuing education, and difficulty maintaining basic needs, such as housing and transportation.  Promote family success in attainment of life skills with a concentration on promoting optimal health to families participating in the Healthy Babies, Kids and Family Program.

Personal Care Attendants (PCA) (apply online) Caregivers needed in the community! CVHHH regularly has Personal Care Attendant (PCA) positions available throughout Washington County to help foster independence for frail and elderly residents. PCA's provide assistance with housekeeping, personal care, hygiene, transportation, and shopping. We provide services 365 days-a-year, 24 hours-a-day, so a wide variety of schedules are often available - from occasional on-call/per diem jobs to a regular full-time 37.5 hour week.
