Home Health Programs & Services in Vermont

Vermont’s 11 not-for-profit Visiting Nurse Associations and home care agencies provide a wide range of high-quality care for people of all ages with acute and long term illnesses. In addition to skilled nursing services, specialty nurses coordinate high-quality individualized care. Licensed Nursing Assistants assist with personal care and activities of daily living. 

Home Care Services Include:

Hospice Services

A specially coordinated home-based program that helps children and adults with a terminal illness, along with their families, cope with death by living life to the fullest. The interdisciplinary team emphasizes care directed toward expert pain and symptom control, maximizing independence and socialization and providing support.
