Development &
Marketing Coordinator - Seeking a
Marketing/PR/Development professional to work with the agency’s
Executive Director and Board of Directors. The position will be
responsible for managing the development and implementation of the
agency’s Marketing Plan, Capital and Direct Mail Campaigns, Fund Raising
Events and community relations.
Marketing/PR/Development
experience is required. Grant writing, prior fundraising experience,
strong interpersonal and database management skills are essential. This
role offers a unique opportunity for professional growth in a dynamic
healthcare environment working with an interdisciplinary team of
professionals and our Board of Directors.
Candidates with a minimum of a
Bachelor’s degree in Marketing, Public Relations or Community Relations

Registered Nurse - In this
position, you will practice,
teach and/or supervise nursing care in a community setting. You will be
responsible for assigned clients and families; carry out nursing skills
contributing to treatment and rehabilitation, prevention of disease and
conservation of health. This
position will be regularly scheduled to work on the weekends during the day,
as well as work the on call rotation during the week and on weekends from
4:30 p.m.-8:00 a.m. Candidate must be currently registered with the state of
Vermont. In this
position you will:
Provide nursing care and interventions in accordance with standards of
practice. Assess clients for appropriateness for home care or hospice
services including assessment of functional status, physical and medical
condition.

Collaborate with client and physician to develop plan of care.
Update plan of care as needed and in accordance with Agency policy. Obtain
physician orders prior to initiation of care and interventions. Initiate
referrals to Agency disciplines and community resources as indicated.
Document provision of care and collaboration with other members of the team
in accordance with Agency policy.
Provide case
management, including data collection, assessment, treatment, teaching,
planning, implementation, and evaluation.
Inform the physician and other disciplines of changes in a client’s
condition and needs.
Demonstrate the ability to prioritize tasks/responsibilities and complete
duties and projects within allotted time. Meet Agency expectations regarding
productivity, weekend, and holiday work and other assignments according to
Agency need.
Demonstrate respect, tolerance and understanding for client’s autonomy and
independence. Act as an advocate for client and family in meeting their
health care needs. Establish trust and confidence in client relationship.
Show good judgment and insight in developing an approach to use within
client relationship. Demonstrate knowledge of and respect for the rights,
dignity and individuality of each client in all interactions.
Assure transitions of care through effective and timely communication,
documentation and planning.
Comply with Medicare Conditions of Participation and other relevant
regulation at all times.
Licensed Nursing Assistant -
Licensed Nursing Assistants (LNA) provide
personal care, emotional support and basic homemaker services in accordance
with the Plan of Care. The LNA works under the direction and supervision of
a Registered Nurse or therapist. This position will provide care daily from
approximately 6:00 a.m.-9:00 a.m. and in the evening from 9-10 p.m. in the
Swanton area. Must have a high school diploma, reliable transportation, and
active LNA license in the State of Vermont. Experience working in a facility
providing care to high needs clients is preferred.
Care Attendant - Our
caregivers help people remain safe and independent at home by providing
homemaking, meal preparation, errands and personal care services. Ideal
candidates are professional, compassionate, reliable, honest, and committed
to providing excellent care and service. Must have the ability to work with
a variety of personalities in a variety of settings. Willing to work
weekends and/or during the week for 4-6 hours in the mornings, afternoons,
or evenings.
