Medicaid Waiver Case Manager - We are seeking an experienced and qualified RN with current knowledge and working familiarity with Medicaid regulations to be responsible for the delivery and coordination of long-term care services to individuals who qualify for Medicaid Waiver services.

This position would:

  • Assess patient physical, emotional and social level of function for eligibility into the waiver program
  • Make referrals to other disciplines as the patient needs indicate
  • Collaborate with agency staff to facilitate continuity of care between home care and long-term care services
  • Coordinate with our Personal Care Attendant Coordinator with annual education requirements for staff
  • Efficiently manage a patient caseload
  • Perform independent living assessments
  • Provide Personal Care Attendant patient introductions and provide PCA supervision at scheduled intervals

Franklin County Home Health Agency

Registered Nurse - Per-diem. Practice, teach and/or supervise nursing care in a community setting; responsible for assigned clients and families; carrying out nursing skills contributing to treatment and rehabilitation, prevention of disease and conservation of health. Registered Nurses participate in Home Care and Hospice programs. 

Requirements: Graduate of an accredited school of nursing; must possess, as a minimum, an Associates Degree in Nursing. BSN preferred.  One year minimum of progressive clinical experience as a registered nurse preferred.  Basic working knowledge of computers preferred. Possess current license as a registered professional nurse in the State of Vermont.

Full-time, weekend and on-call rotation required practice, teach and/or supervise nursing care in a community setting; responsible for assigned clients and families; carry out nursing skills contributing to treatment and rehabilitation, prevention of disease and conservation of health.  Participate in the Hospice Program.

CCare Attendant -
Per-diem. Use your skills and talents to help people as a Care AAttendant in our Long Term Care Department.  You will make a positive impact on ptpatient's abilities to function independently at home. Care Attendants assist people in ththeir homes by providing light housekeeping, cooking grocery shopping, errands, and s home personal care.  We are looking for people who like to work mornings, evenings orweekends.

PACE
Vermont Inc. - Program of All-Inclusive Care for the Elderly

PACE Vermont serves individuals who are aged 55 or older, certified by their state to need nursing home care, able to live safely in the  community at the time of enrollment, and live in a PACE service area.

RN - Part-time BS in nursing with current nursing experience with at least one year’s experience working with frail, older adults.  Recent experience in home health is desirable.  This job is for the Rutland Center
