Registered Nurse - Seeking compassionate, team-oriented individuals to provide a full range of professional nursing services to clients of all ages including admission, evaluation and care plan development. The individual is also responsible for explaining the hospice program and philosophy, as well as for admitting, evaluating and developing care plans for all qualified clients. On-Call Hospice Nurse - Experienced, team-oriented professionals to provide skilled nursing,
intervention and supportive care to clients throughout Rutland County
for evening and night on-call hours. Candidates will assess, admit and
establish plans of care based on physician orders and the
interdisciplinary team plan of care. On-call hours are 4 p.m. to 8 a.m.
Monday through Friday and 8 a.m. to 8 a.m. Saturday and Sunday. Physical Therapist (22.5 hrs/wk) - We’re seeking an enthusiastic, team-oriented individual to provide physical therapy in the home environment to our adult and geriatric population. The individual will participate in evaluating and admitting patients, formulating treatment plans, evaluating patients’ progress, and reassessing patients on an ongoing basis. As a member of a multi-disciplinary team, this individual will participate in the instruction and supervision of agency staff to assist in carrying out physical therapy and participate in the instruction of in-service programs. Candidates must be certified to practice Physical Therapy in Vermont, and have a bachelor’s degree or master’s degree with 3-4 years experience. The successful candidate will have excellent interpersonal, communication and assessment skills.
